Download PDF Medical Release &
Code of Conduct

For the safety of our children/youth, a Medical Release and Code of Conduct Form is required to participate in fellowship events or attend camp. Please fill out the form (make sure to complete both pages) and return it to the Youth Office. If you bring a friend to an event, they will need a form as well. However, you only need to complete one form per year. If you are not sure if you have one on file, please call Debbie in the Youth Office.

Inclement Weather Policy
Inclement weather policy for all First Lutheran Church events: If the Sioux Falls School District has cancelled classes or evening activities, First Lutheran Church events involving youth will be cancelled as well. Also, Wednesday Night Meals will not be served. Adult activities will be cancelled at the discretion of the group leader. Cancellation information for all events will be available on the website (www.flcsf.org), Facebook page (www.facebook.com/flcsf), and via phone (605) 336-3734. Information will be submitted to local radio and television stations.
