Medical Release &
Code of Conduct
For the safety of our children/youth, a Medical Release and Code of Conduct Form is required to participate in fellowship events or attend camp. Please fill out the form (make sure to complete both pages) and return it to the Youth Office. If you bring a friend to an event, they will need a form as well. However, you only need to complete one form per year. If you are not sure if you have one on file, please call Kristy in the Youth Office.
